Study Comparing Two Types of Radiation Therapy for Newly Diagnosed Gynecologic Cancers

This clinical trial is comparing the side effects of two different types of radiation therapy for patients with newly diagnosed endometrial or cervical cancer. Participants will be randomly assigned to receive either conventional photon radiation or proton radiation, both of which are standard treatments.

Program Overview
The aim of this study is to compare the side effects of two standard radiation treatments for endometrial and cervical cancer. Participants will be randomly assigned to receive either conventional photon radiation or proton radiation. This study will help determine which type of radiation therapy has fewer side effects for patients.
Eligibility Criteria
Inclusion Criteria
- Newly diagnosed with endometrial cancer after surgery and planning to receive chemotherapy and radiation, or cervical cancer planning to receive chemoradiation with additional brachytherapy.
- Cancer must be confirmed by a lab test.
- Must be 18 years or older.
- Must be in relatively good health, with a performance status score of 2 or less.
- Must be able to understand and sign a consent form, or have someone who can do so on their behalf.
- Must have insurance approval for proton therapy.
Exclusion Criteria
- Cancer has spread beyond the para-aortic lymph nodes.
- Remaining tumor larger than 2 cm after surgery for endometrial cancer.
- Advanced cervical cancer planning to receive pembrolizumab.
- Unable to receive chemotherapy for cervical cancer.
- Previous radiation to the pelvic area.
- Currently receiving other experimental treatments.
- Diagnosed with carcinosarcoma.
- Poor kidney function with creatinine clearance less than 30 mL/m2.
- Unable to lie flat or tolerate radiation therapy.
- Refusal to sign the consent form.
- Any other active cancer that could affect the study results.
