This clinical trial is comparing two different radiation treatments, radioembolization and external radiation therapy, to see which is more effective and better tolerated for treating liver cancer.

Program Overview

The study aims to compare the effectiveness and safety of two radiation treatments for liver cancer: transarterial radioembolization (TARE) and stereotactic body radiation therapy (SBRT). In the SBRT group, patients will receive standard radiation therapy over 3 to 5 sessions, with doses adjusted for safety. In the TARE group, patients will undergo a procedure to deliver a radioactive substance, Yttrium-90, directly to the liver tumor through the blood vessels. The main goal is to see how often the treated tumor needs additional treatment within 12 months.

Eligibility Criteria

Inclusion Criteria

  • Able to provide written consent and agree to follow study procedures.
  • Available for the entire duration of the study.
  • Male or female, 18 years or older.
  • Have no more than 3 liver cancer lesions suitable for TARE or SBRT.
  • Child-Pugh score of 8 or less, indicating liver function.
  • ECOG performance status of 2 or less, indicating general health.
  • Adequate organ function, with specific blood test results:
    • Bilirubin level less than 4.0 mg/dL.
    • Albumin level greater than 2 g/dL.

Exclusion Criteria

  • Previous SBRT or radioembolization treatment to the target tumor.
  • Large blood vessel invasion by the tumor.
  • Planned or recommended additional systemic therapy.
  • Pregnant or breastfeeding women. Women who can become pregnant must have a negative pregnancy test within 14 days before joining the study.
  • Severe allergic reaction to iodinated contrast used in imaging tests.
  • Blood clotting issues that make transarterial therapy unsafe.
