Program Overview
As people live longer after prostate cancer treatment, maintaining quality of life, including sexual health, becomes important. This study is testing a new radiation therapy method called stereotactic body radiotherapy (SBRT), which uses high doses of radiation. At UCLA, SBRT can be delivered using advanced imaging machines. The trial aims to find the best way to protect the nerves and blood vessels crucial for erectile function using a technique called neurovascular sparing. This involves using MRI images to map these important structures and adapt the radiation treatment to each patient's anatomy before each session. While this method is standard and low-risk, it is complex and requires a team of experts. The study will determine if this approach reduces side effects.
Description
Radiation therapy for prostate cancer can sometimes harm the blood vessels and nerves around the prostate that are important for normal erectile function. These structures are close to the prostate and often receive some radiation during treatment. In the past, larger treatment areas were used to ensure cancer control, but this increased the risk of side effects. With modern technology, it's now possible to target the cancer more precisely and spare these sensitive areas from high doses of radiation. This study uses a technique called neurovascular-sparing, which combines MRI images with standard radiation planning to accurately identify and protect these structures. Daily adjustments to the treatment plan may also help ensure the cancer is treated effectively while minimizing harm to these areas. The study will explore the outcomes of using this approach, as specific guidelines for protecting these structures are not yet well established.
Eligibility Criteria
Inclusion Criteria
- Must be 18 years or older.
- Have a confirmed diagnosis of localized prostate cancer.
- Undergo recommended staging tests based on risk, including advanced imaging if needed.
- No signs of cancer spread to distant lymph nodes, bones, or organs.
- Have a performance status score of 2 or less, indicating good overall health.
- Able to undergo MRI of the pelvis.
- No urgent need for radiation treatment.
- Provide written informed consent and agree to follow study requirements.
Exclusion Criteria
- Have neuroendocrine or small cell prostate cancer.
- Have evidence of cancer spread beyond local lymph nodes.
- Have cancer in specific areas of the prostate as shown by tests.
- Have had certain prostate treatments like cryosurgery or brachytherapy.
- Have had previous pelvic radiation.
- Have a history of certain bowel diseases or genetic conditions.
- Have a penile implant before treatment.
