Study on Tucatinib or Placebo with Trastuzumab and Pertuzumab for Advanced HER2+ Breast Cancer

This clinical trial is testing whether the drug tucatinib, when combined with trastuzumab and pertuzumab, is more effective than a placebo in treating HER2-positive breast cancer that has spread or cannot be surgically removed. The study will also monitor any side effects from this combination of drugs.

Program Overview
The purpose of this study is to find out if tucatinib works better than a placebo when used with other drugs to treat HER2-positive breast cancer that has spread or cannot be removed by surgery. A placebo is a pill that looks like the real drug but has no active medicine. All participants will receive either tucatinib or a placebo, along with trastuzumab and pertuzumab, which are standard treatments for this type of cancer. Participants will be randomly assigned to a group, and neither they nor their doctors will know which group they are in.
Description
Participants in this study will be divided into two groups:

Control Group: Participants will take a placebo pill twice a day and receive trastuzumab and pertuzumab every 21 days.

Experimental Group: Participants will take tucatinib 300 mg twice a day and receive trastuzumab and pertuzumab every 21 days.
Trastuzumab and pertuzumab will be given as follows:

Trastuzumab can be given through an IV at a dose of 6 mg/kg or as a shot under the skin at a fixed dose of 600 mg, once every 21 days.

Pertuzumab will be given through an IV at 420 mg every 21 days, or as a fixed dose combination with 600 mg pertuzumab, 600 mg trastuzumab, and 20,000 units of hyaluronidase as a shot under the skin, once every 21 days, instead of giving trastuzumab and pertuzumab separately.
Eligibility Criteria
Inclusion Criteria
- Have HER2-positive breast cancer confirmed by specific medical guidelines.
- Have cancer that cannot be surgically removed or has spread to other parts of the body.
- If the cancer has returned after previous treatment, there must be at least a 6-month gap since the last treatment with trastuzumab and pertuzumab.
- Have completed 4-8 cycles of initial therapy with trastuzumab, pertuzumab, and taxane without disease progression.
- Have known hormone receptor status, which can be positive or negative.
- Have a performance status score of 0 or 1, indicating full activity or some restriction in strenuous activities.
- Based on a brain MRI, participants may have:
- No brain metastases
- Untreated, asymptomatic brain metastases not needing immediate treatment
- Previously treated, asymptomatic brain metastases that have not progressed since treatment
Exclusion Criteria
- Have previously been treated with any HER2 or EGFR-targeting tyrosine kinase inhibitors, except for neratinib under specific conditions.
- Cannot undergo a contrast-enhanced MRI of the brain.
- Based on brain MRI and clinical assessment, participants may not have:
- Symptomatic brain metastases after local treatment
- Progression of brain metastases since starting initial therapy
- Ongoing use of systemic corticosteroids above a certain dose
- Untreated brain lesions in risky locations
- Known or suspected leptomeningeal disease
- Poorly controlled seizures or other persistent neurological symptoms
