This clinical trial is comparing the effectiveness of a new treatment using nivolumab and ipilimumab against the current standard treatments, sorafenib or lenvatinib, in patients with advanced liver cancer who have not received previous systemic therapy.

Program Overview
The purpose of this study is to see if a combination of two drugs, nivolumab and ipilimumab, can help people with advanced liver cancer live longer compared to the usual treatments, sorafenib or lenvatinib. This study is for patients who have not yet received any systemic treatment for their liver cancer.
Eligibility Criteria
Inclusion Criteria
- Have a confirmed diagnosis of advanced liver cancer (HCC).
- Have at least one tumor that can be measured and has not been treated before.
- Have a Child-Pugh score of 5 or 6, which is a measure of liver function.
- Be in good overall health with an ECOG performance status of 0 or 1, meaning fully active or restricted in physically strenuous activity but ambulatory.
Exclusion Criteria
- Have a specific type of liver cancer known as fibrolamellar, sarcomatoid, or mixed with bile duct cancer.
- Have had a liver transplant.
- Have had episodes of severe confusion or coma due to liver disease in the past year.
- Have active cancer that has spread to the brain or the membranes covering the brain and spinal cord.
