Program Overview

The EXPAND trial is a research study aimed at finding out if using targeted treatments on all cancer sites can help patients with pancreatic cancer that has spread to a few areas (oligometastatic disease) live longer and delay the progression of their disease. The study compares this approach to using only standard systemic therapy.

Description

The main goal of this study is to see if treating all cancer sites with targeted therapies, like surgery or radiation, can help patients with pancreatic cancer that has spread to a few areas live longer without their disease getting worse, compared to using only standard treatments.

Secondary goals include:

  • Checking if this approach improves overall survival.
  • Evaluating the safety and side effects of targeted treatments.
  • Measuring the time until new cancer spots appear and the time until patients need new treatments.
  • Comparing the quality of life between different treatment groups.
  • Exploring if certain biological markers can predict who will benefit most from targeted treatments.

The study will also look at how these treatments affect the cancer environment and identify any changes in biological markers over time.

Eligibility Criteria

Inclusion Criteria

  • Must be 18 years or older.
  • Have a confirmed diagnosis of stage IV pancreatic cancer.
  • Be in good overall health with an ECOG performance status of 0 to 2.
  • Be a candidate for targeted treatments to all cancer sites, including those with limited spread.
  • Have between one and five cancer spots that can be treated locally.
  • Provide recent imaging scans showing limited spread of cancer.
  • Women of childbearing potential must not be pregnant or breastfeeding and must use effective contraception.
  • Men must use effective contraception and avoid fathering a child during the study and for three months after.
  • Have adequate organ function as shown by recent lab tests.

Exclusion Criteria

  • Have fluid buildup in the chest or abdomen due to cancer.
  • Have cancer spread to the brain or spinal fluid.
  • Have cognitive impairments that prevent understanding the study.
  • Have any condition that could interfere with the study or its results.
  • Have extensive bone marrow involvement.
  • Have had more than four previous treatments for metastatic cancer.
  • Have active autoimmune diseases that prevent safe delivery of radiation.
  • Have other serious health conditions or cancers that could affect participation.
