This study is testing a new method of performing radiosurgery on brain tumors that aims to protect important brain functions such as memory, language, and attention. By using advanced imaging techniques, doctors can identify and avoid critical areas of the brain during treatment. The goal is to reduce the risk of cognitive decline after treatment in patients with 1 to 3 brain metastases.

Description

Brain metastases are tumors that have spread to the brain from other parts of the body and affect about one-third of adult cancer patients. The standard treatment for patients with a limited number of brain metastases is a type of precise radiation called stereotactic radiosurgery (SRS). However, this treatment can sometimes lead to cognitive decline, affecting memory and other brain functions.

This study aims to test a new approach to SRS that uses advanced imaging techniques to protect important brain areas. These techniques help doctors see and avoid critical white matter tracts and the hippocampus, which are important for memory and other cognitive functions. By sparing these areas from high doses of radiation, the study hopes to improve cognitive outcomes for patients.

The study will enroll 60 adult patients with 1 to 3 brain metastases. Participants will undergo MRI scans before and after treatment to monitor changes in the brain. Cognitive tests will be conducted to assess memory and other functions before and after treatment. The results will be compared to historical data to see if the new approach leads to better cognitive outcomes.

Eligibility Criteria

Inclusion Criteria

  • Adults aged 18 years or older
  • Have 1 to 3 brain metastases, each smaller than 3 cm
  • Good overall health with an ECOG performance status of 0 to 2
  • Able to participate in cognitive testing
  • Expected to live more than 6 months
  • Confirmed cancer diagnosis from another part of the body
  • Willing and able to have MRI scans
  • Able to provide informed consent

Exclusion Criteria

  • Pregnant or nursing women
  • Women who can become pregnant and are not using contraception
  • Unable to have an MRI with contrast
  • Tumor invading critical brain areas
  • Planning to have chemotherapy on the day of SRS
  • Previous whole brain radiation therapy
  • Cancer spread to the brain's lining (leptomeningeal metastases)
  • Metastases from certain types of cancer like germ cell tumors or small cell carcinoma
